(they come 4 cards to a sheet), as well as the address you wish the blank
ID cards mailed to. The blank cards along with a Service Request Form
(K1497) will be shipped to you.
STEP
TWO - PROCESSING ID CARDS
- Do not cut blank ID cards out of sheet (return
them whole).
- Fill out the cards with the appropriate information
- If errors are made, do not use white out or
correction markings of any kind. Mark VOID and use another blank ID
card.
STEP THREE - PHOTOS
- You are required to supply the photos for the IDs. Good Polaroids
are acceptable.
- Head size of photo should be 2 cm from top of the head to chin.
- Subject should be on a white background preferably.
- Do not put subject too close to background when taking photo, as the
shadow from the flash will intensify. Allow lots of area to show around
subject (it will be trimmed later).
- If using a 135 mm camera, with a standard lens (50 mm), subject should
be 12ft from camera.
- Legibly print the name of the individual on the back of photo
using a quick drying pen.
- If quality of photo is poor, or if photos are the wrong size,
they will be returned and further time delays will be incurred..
- DO NOT TRIM PHOTO, OR ATTACH TO CARD.
STEP FOUR - RETURN SHIPMENT
- Complete Service Request Form (K1497) and include
your email address on it.
- Ship Service Request Form, photos along with
ALL ID CARDS (including blank and void ones) in an envelope marked "Photos
- Do Not Bend" on both side of the envelope
- Mail package to address above (recommend
Express Post)
- PAYMENT OF A FEE IS NO LONGER REQUIRED
(March, 2007)
